Advocacy
Our Care Advocates educate and engage patients regarding their health and wellness needs. This includes everything from developing and improving healthy eating habits, incorporating movement and physical activity into a daily routine, and encouraging patients to stay engaged with their hometown provider. Our Care Advocates are at the core of what we do and our ability to provide high-quality healthcare to patients between visits with their provider.
Transitional Care
Management
When a patient is discharged from the hospital, they may need assistance transitioning back into their home setting. To ensure a continuity of care for both the patient and their caregiver, Transitional Care Management (TCM) serves as the interactive support to drastically reduce risk of relapse and reduce the likelihood of a hospital readmission.
Polychronic Health
Management
Polychronic Health Management is the process of caring for patients who may have been diagnosed with two or more chronic conditions, such as diabetes, Hypertension, CKD, etc. This subset of patients, which continues to grow every year, is expected to grow to an estimated 83.4 million people by 2030 (compared to 30.8 million in 2015). The management of polychonic conditions gives local providers across different specialties the opportunity to collaborate with one another and bridge gaps in care in with patient-centered care at the forefront of polychronic health management delivery.
Kidney Health
Education
Through our proprietary kidney health education tools, we teach patients the best and most efficient ways to independently manage their kidney health. Topics covered include information such as how to read a medication label, managing your salt intake, kidney-friendly recipes, kidney transplant education, and dialysis modality education.
Social Services
Coordination
Treating the entire patient includes connecting patients to external resources and assisting with tasks such as finding a place to live, access to food and transportation, or getting medical care. Our Care Advocates connect patients with our Social Worker and Dietician team to help, because we understand that Social Determinants of Health pay a critical role in understanding total patient care.
Nutrition
Services
Nutrition services are a cost-efficient way to prevent or manage many chronic diseases such as diabetes, high blood pressure and chronic kidney disease. Our dietitians provide telehealth support while helping patients find foods that they enjoy that fit within their diet recommendations. A referral to a dietitian can help free up time for other healthcare providers and offer patients additional support to achieve their health goals effectively.
